Patella (Kneecap) and Knee Fractures: Symptoms, Surgery, and Recovery Process

The knee joint is the most load-bearing, mobile, and complex hinge in our body. A direct impact from falling onto your knee, a "dashboard injury" during a car crash, or a simple loss of balance in older adults can easily result in a fracture of the kneecap (Patella) or the surrounding bones that form the knee joint. In such cases, a patient's biggest fear is usually, "Will I ever be able to bend my knee again?" At DMN Orthospine Clinic in Istanbul, our flawless anatomical surgical techniques and early physical therapy protocols ensure our patients return to their active lives without suffering from permanent joint stiffness.

How is a Patella (Kneecap) Fracture Diagnosed?

The patella is a shield-shaped bone located at the very heart of the mechanism that allows our thigh muscles to keep the leg straight. When it breaks, the following symptoms occur:

  • Rapid, balloon-like swelling over the kneecap (due to internal bleeding within the joint, known as hemarthrosis).
  • A palpable gap, depression, or a feeling of shattered bone fragments right over the knee.
  • Severe, sharp pain and an absolute inability to bear any weight on the leg.
⚠️ Critical Test: Can You Do a Straight Leg Raise?

When a patient arrives at our clinic with a kneecap trauma, the first test we perform while they are lying flat on their back is asking them to lift their heel straight up off the bed. If you cannot perform a straight leg raise, it indicates that your kneecap is structurally separated and your "Extensor Mechanism" (the system that keeps your leg straight) is ruptured. This situation requires definitive and urgent surgical intervention.

Can a Kneecap Fracture Heal Without Surgery? (Bracing/Casting)

Yes, not every kneecap fracture requires surgery. If the X-rays show that the bone fragments have not separated at all (non-displaced), and most importantly, if the patient can lift their leg completely straight into the air, non-surgical treatment is applied.

The patient is provided with a long leg cast or, more commonly today, a hinged knee brace locked in a fully straight position. During this 4 to 6-week period, it is vital to perform isometric (muscle-clenching) exercises inside the brace to prevent severe muscle wasting (atrophy).

Surgery for Patella and Knee Fractures (Wires and Plates)

If the bone fragments have pulled apart or if there is an uneven "step-off" on the smooth joint surface, surgery is mandatory. An uneven joint surface will inevitably lead to severe knee arthritis (osteoarthritis) in the future. Board-certified surgeons Dr. Sedat Duman and Dr. Muhammed Duman apply the following modern techniques:

1. Tension Band Wiring

This is the most widely used, classic, and robust method globally for patella fractures. The separated bone pieces are brought together and secured using fine steel pins (K-wires) and flexible stainless-steel wire wrapped in a figure-8 pattern. This brilliant biomechanical system converts the pulling forces that occur when you bend your knee into compressive forces, pushing the broken pieces tightly together.

2. Plates and Screws (For Periarticular Knee Fractures)

If the fracture is not in the kneecap itself, but in the lower end of the thighbone (Distal Femur) or the upper end of the shinbone (Tibial Plateau) that form the knee joint, specialized anatomical titanium plates and screws are used to flawlessly reconstruct the joint surface.

3. Partial Patellectomy (Removal of Small Fragments)

If the very top or bottom edge of the kneecap is shattered beyond repair (comminuted), these tiny unsalvageable bone crumbs are surgically removed. The main tendon is then reattached directly to the remaining solid kneecap bone using highly durable sutures.

Recovery After Surgery: When Can I Bend My Knee?

The greatest enemy of any knee joint fracture is joint stiffness (arthrofibrosis). As the bone heals, the joint capsule can scar and adhere, making it impossible to bend the knee.

At DMN Orthospine, our surgical fixation is so secure that, with your doctor's permission, physical therapy begins just a few days or a week after surgery. Using specialized machines (CPM - Continuous Passive Motion) or under the guidance of a physiotherapist, the knee is slowly and safely pushed to bend. Patients who strictly adhere to their physical therapy program typically regain almost full knee bending capacity within a few months.

Frequently Asked Questions (FAQ)

Can a kneecap fracture heal without surgery?

If the bone fragments are not separated (non-displaced) and the patient can lift their leg completely straight into the air (intact extensor mechanism), it can heal without surgery using a long leg cast or a hinged knee brace within 4 to 6 weeks.

Will I be able to bend my knee normally after surgery?

The biggest enemy of knee joint fractures is joint stiffness. Because modern surgical fixation is very secure, patients begin knee bending exercises (physical therapy) shortly after surgery. Patients who strictly follow their physical therapy program usually regain almost full knee bending capacity.

Are the surgical wires or pins removed from the kneecap later?

Because the kneecap is located directly under the skin, the steel wires and pins used in surgery can sometimes cause irritation, especially when kneeling. If they cause discomfort, they can be easily removed with a minor surgical procedure once the bone has fully healed (typically after 1 year).

When can I bear weight on my leg after a patella fracture?

Depending on the fracture type and surgery, you are often allowed to stand and bear controlled weight immediately, provided your knee is locked in a fully straight (extension) position using a specialized brace. However, bearing weight while the knee is bent may be restricted for several weeks.

Improper treatment or delayed physical therapy for knee joint fractures can lead to permanent disability and stiffness. For flawless reconstruction of your joint surface and a safe recovery process, you can consult the expert orthopedic surgeons at DMN Orthospine Clinic in Istanbul.
